After reviewing Module 6: Lecture Materials & Resources, explain approaches to community intervention and evaluation. Implement a teaching and evaluation plan.
1. Choose one topic from the prevention strategies and intervention recommendations identified in Module 4 Assignment.
2. Develop a teaching plan, including objective, content outline, teaching method, and time in a table format found in slide 3 of the Template Download Template.
3. Develop an evaluation plan for your teaching intervention and create a tool for feedback from your learners. Be sure to include the questionnaire and any other teaching tools within the powerpoint presentation for faculty analysis.
4. Implement the teaching plan and evaluation during Visit 5 of your clinical experience.
5. Summarize the outcomes of your teaching plan and evaluation based on the results from the questionnaires.

This learning activity aims for a full understanding and unbiased view of the family—not just its problems, but also its strengths, values, and goals.
Understanding family structure and style is essential to caring for a family in the community setting. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

• Upon completion of the interview, write a 750-1,000-word paper.
• Analyze your assessment findings and the family’s answers to your questions.

This family health assessment is a two-part assignment. The information you gather in the first part of the assignment will be utilized for the second part of the assignment.
1. Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment.
2. Document the responses as you conduct the interview. Do not put the family’s name, but utilize initials, the gender of each family member, and their age.
Your paper must include the following:
• Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
• Summarize the overall health behaviors of the family. Describe the current health of the family.
• Based on your findings, describe at least two of the functional health pattern strengths noted in the findings.
• Discuss three areas in which health problems or barriers to health were identified?
• Your assignment must include an eco-map and a genogram – as shown in the required textbook.

Approaches to community intervention and evaluation:
Community intervention is a method that aims to promote health, prevent disease, and improve the quality of life for individuals, families, and communities. One of the primary approaches to community intervention is the ecological model, which emphasizes that the individual is affected by multiple levels of influence, including the individual, interpersonal, organizational, community, and policy levels. Another approach is community-based participatory research, which involves the collaboration of community members, researchers, and other stakeholders to address community health issues. Community interventions can also be evaluated using different methods, such as process evaluation, outcome evaluation, and impact evaluation. Process evaluation assesses how the intervention is implemented and whether it is reaching the target population. Outcome evaluation determines whether the intervention achieved its intended outcomes, while impact evaluation assesses the broader effects of the intervention on the community.

Teaching plan and evaluation plan for preventing falls in older adults:
Objective:
To educate older adults on the risk factors for falls and strategies to prevent falls.

Teaching method:

Evaluation plan:
The evaluation plan will include a pre-test and post-test questionnaire to assess the participants’ knowledge and attitudes towards falls prevention. The questionnaire will include multiple-choice and open-ended questions, such as “What are the risk factors for falls?” and “What strategies can you implement to prevent falls?”. Participants will also be asked to provide feedback on the teaching method and content. The questionnaire will be distributed before and after the presentation.

The outcome evaluation will be based on the results from the pre-test and post-test questionnaire. The data will be analyzed using descriptive statistics and paired t-tests to determine the effectiveness of the teaching intervention. The feedback from the participants will also be analyzed to improve the teaching method and content.
