• Briefly explain the neurobiological basis for PTSD illness.
• Discuss the DSM-5-TR diagnostic criteria for PTSD and relate these criteria to the symptomology presented in the case study. Does the video case presentation provide sufficient information to derive a PTSD diagnosis? Justify your reasoning. Do you agree with the other diagnoses in the case presentation? Why or why not?
• Discuss one other psychotherapy treatment option for the client in this case study. Explain whether your treatment option is considered a “gold standard treatment” from a clinical practice guideline perspective, and why using gold standard, evidence-based treatments from clinical practice guidelines is important for psychiatric-mental health nurse practitioners.

The neurobiological basis for PTSD illness is thought to involve changes in the hypothalamic-pituitary-adrenal (HPA) axis and the noradrenergic system. These changes may lead to increased reactivity of the stress response and changes in the brain regions involved in memory, emotion regulation, and fear processing.
The DSM-5-TR diagnostic criteria for PTSD include exposure to a traumatic event, symptoms of intrusion, avoidance, negative alterations in cognition and mood, and alterations in arousal and reactivity. The video case presentation does provide sufficient information to derive a PTSD diagnosis as the client describes experiencing a traumatic event and exhibiting symptoms such as flashbacks, avoidance, and emotional numbing. The video case presentation also includes other diagnoses such as depression and alcohol use disorder, which may be comorbid conditions that occur alongside PTSD.
The other psychotherapy treatment option for the client in this case study could be cognitive-behavioral therapy (CBT). This treatment is considered a “gold standard” treatment from a clinical practice guideline perspective as it has strong empirical support for its effectiveness in treating PTSD. Using gold standard, evidence-based treatments from clinical practice guidelines is important for psychiatric-mental health nurse practitioners as it ensures that clients receive the most effective and up-to-date treatment options.
