Week 1 – AssignmentWhere is My Chart Read “Where’s My Chart?” located in Chapter 1 of the text (p.15), then prepare a paper discussing and relating your similar or another individual’s similar experiences to that of the patient in the story.  Address the following question: Why is Health Care Record adoption/initiation so far behind other health-care industry related change/implementation initiatives?  Review The Office of the National Coordinator for Health Information Technology, HiMSS  and The Medical Records Institute, then prepare a two to three page APA (6th edition) formatted response (excluding title and reference page) to “Where’s My Chart?” and why Electronic Health Record  initiatives are so far behind other industries in health care.  Locate updated items on the Website of the Office of National Coordinator of Health Information Technology and various leading health care organizations.  Include a minimum of two APA citations from scholarly sources in addition to the course text. Carefully review the Grading Rubric for the criteria that will be used to evaluate your assignment.
